29 CFR 1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program which included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):     a) Advanced Distribution Systems, Inc., 614 Route 303, Blauvelt, NY - Production Department:   On Target Staffing LLC did not develop, implement and maintain a written hazard communication program for On Target Staffing LLC for employees who were handling containers of hazardous chemicals [e.g., applying heat to shrink wrap Rita Hazan Root Concealer (Flammable Aerosol, etc.)] during packaging operations; on or about 3/15/19.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    a) Advanced Distribution Systems, Inc., 614 Route 303, Blauvelt, NY - Production Department:   On Target Staffing LLC employees were handling containers of hazardous chemicals [e.g., applying heat to shrink wrap Rita Hazan Root Concealer (Flammable Aerosol)] during packaging operations and On Target Staffing LLC did not provide their employees who worked at this location with information and training on the requirement of the OSHA Hazard Communication Standard, labels, safety data sheets, generic training on hazardous chemicals and did not verify that the host employer provided specific training on the hazards of these chemicals; on or about 3/15/19.
