29 CFR 1926.451(f)(1): Scaffold and/or scaffold components were loaded in excess of their maximum intended loads or rated capacities, whichever was less: Jobsite: The two (2) Werner type II extension ladders each equipped Qualcraft Model Z420 aluminum rung style jacks used to support the scaffold platform spanning were loaded in excess of their maximum intended loads and rated capacities. On or about March 21, 2019, one of the Werner ladders failed resulting in three (3) employees to fall approximately 11 feet to the ground with one employee sustaining serious injuries.
