29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: a) On or about 02/06/19 in the yard truck delivery area; employees were exposed to an unprotected fall from a height of between 53" - 60" from tractor trailer truck beds while they were engaged in the unloading of wooden crates containing glass panels. ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: a) On or about 02/06/19 in and around the yard area and truck unloading; employees were exposed to being struck by wooden crates of glass weighing between 500 - 1,000 pounds when powered industrial trucks (PIT) used boom extensions, such as but not limited to the Caldwell brand fork lift boom (model PB 60 - serial 94217), to lift these wooden crates off of the bed of tractor trailers and name plate on the Toyota PIT ( serial number 10484) did not have the information related to use of the lift boom according to the manufacturer's instructions or approval. ABATMENT CERTIFICATION REQUIRED
