29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around the skylight. On or about March 26, 2019 at 8055 Browns Ford Road, Greensboro, Georgia an employee removing old and installing new skylight fell approximately 45' - 7" to the lower level when he fell through a skylight when not tied off.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii). On or about March 26, 2019 at 8055 Browns Ford Road, Greensboro, Georgia an employee that had been on site for three (3) days, working on a roof approximately 45 feet and 7 inches above the lower level, removing old skylights and installing new skylights not been trained in the applications and needs for fall protection.
