29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift. On the west side of the construction site, employees working without personal fall protection in an elevated aerial lift over 12 feet to the roof were exposed to an unprotected fall hazard.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On the west side of the construction site, employees installing roof caps on the roof of a building without fall protection were exposed to unprotected fall hazards of approximately 40 feet to the ground below.
