On or about March 21, 2019 and at times prior thereto, an employee was required to wear a respirator without being medically evaluated on an annual basis during tasks including but not limited to painting operations, exposing the employee to the hazardous of respirator use.

29 CFR 1910.1025(g)(1): Appropriate protective work clothing and equipment were not used when employee(s) were exposed to lead above the permissible exposure limit (PEL), without regard to the use of respirators, or where the possibility of skin or eye irritation exists. On or about March 21, 2019 and at times prior thereto, the employer did not require employees that were susceptible to skin irritation while using LOCTITE and KESTER Lead based soldering wire to wear protective gloves, exposing employees to the hazards of Lead.

29 CFR 1910.1025(h)(1): All surfaces were not maintained as free as practicable of accumulations of Lead. On or about March 21, 2019 and at times prior thereto the employer did not ensure surfaces such as but not limited work mats at soldering stations were free of accumulations of Lead. Wipe samples taken on soldering stations showed areas to be above the U.S. Department of Housing and Urban Development (HUD) guidelines of 21.53 ug/100cm2, exposing employees to the hazards of Lead.

29 CFR 1910.1025(l)(1)(ii): The employer did not train each employee who is subject to exposure to lead at or above the action level, or for whom the possibility of skin or eye irritation exists, in accordance with the requirements of 29 CFR 1910.1025. On or about March 21, 2019 and at times prior thereto, employees working in soldering areas where there is a potential for skin irritation due to exposure to Lead Solder were not provided or informed of the hazards of Lead and content in Appendices A and B of 29 CFR 1910.1025 of the Occupational Safety and Health Standards for Lead, exposing employees to the hazards of Lead.
