29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who was on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: a) Jobsite: On March 19, 2019 and at times prior thereto an employee performing steel erection activity which included installing metal roof accessories, was not using a fall protection system. The employee was exposed to a 17 foot fall to the ground below. Note) Certification of corrective action is required for this item.

29 CFR 1926.761(b): The employer did not train each employee exposed to a fall hazard in accordance with the requirements of this section. The employer did not institute a training program and ensure employee participation in the program: a) Jobsite: On March 19, 2019 and at times prior thereto the employer had not established and implemented a training program which addressed specific hazards and requirements of performing steel erection activities. An employee installed roof decking accessories on a systems engineered metal building with out a fall protection system in place. The employee was not familiar with the fall protection requirements of the steel erection standard for the work that was being done. Note) Certification of corrective action is required.
