29 CFR 1910.106(b)(6): In locations where flammable vapors may be present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition. Sources of ignition may include open flames, lightning, smoking, cutting and welding, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat. a) At the facility, in the production area, on or about March 4, 2019, the employer did not take precautions to prevent the use of a "torpedo heater," in the proximity of highly flammable materials, exposing employees to fire and smoke inhalation hazards. b) At the facility, in the production area, on or about March 4, 2019, the employer did not take precautions to prevent the use on or storage of a gasoline container in the proximity of a battery charger, exposing employees to fire and smoke inhalation hazards.

29 CFR 1910.157(g)(2): The employer provided portable fire extinguishers for employee use in the workplace, without providing employees the education required in paragraph (g)(1) of this section upon initial employment and at least annually thereafter. a) At the facility, in the production area, on or about March 19, 2019, the employer did not train employees on the basic principles of fire fighting to include, but not limited to, recognizing insipient fire or types of fires, and best practices for fighting the specific types of fire, thus exposing employees to burn and smoke inhalation hazards.
