29 CFR 1926.416(a)(1): Employee(s) were permitted to work in proximity to electric power circuits that were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    a) C-Level Switchgear Room - An employee was attempting to move 480V 3 phase cables from a higher temporary hole in the wall to a lower permanent location without de-energizing or inspecting the cables prior to beginning the task. One or more of the cables came in contact with the wire mesh cable tray that they were supported in causing the cables to arc flash, on or about March 14, 2019.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
