29 CFR 1910.25(b)(1): The employer did not ensure that handrails, stair rail systems, or guardrail systems were provided in accordance with 29 CFR 1910.28: A. On or about May 2, 2019, Loading Dock C: A 7-riser stairway from the emergency exit to the lot was not provided with a standard guardrail. The stairs measured 44-inches wide and the top landing was 52-inches from the ground. The stairway was open on both sides. No intermediate rail was provided to prevent employees and truck drivers from falls.

29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening whwere they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910: A. On or about May 2, 2019, Secondary Department: A 120V electrical panel, identified as Panel V1, had a wooden work table installed in the working space in front of it. The panel powered outlets and lights in the area. The table extended 30 1/2 inches from the front of the panel. Table S-1 of Subpart S requires 36-inches of clear space in front of electrical with 0-150 nominal volts to ground.

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o): A. On or about May 2, 2019, Double Torsion: Machine #13 had an unguarded belt and pulley at the back edge. The equipment was run in automatic mode and was used to make clipboard springs. The top pulley was 9-inches in diameter, the bottom was 4-inches in diameter and they were spaced 25-inches apart on center and used a 1/2-inch wide flat belt. Employees were exposed to laceration hazards from the unguarded ingoing nip points.
