29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about 03/26/2019 at the Friendly's Restaurant Re-Roofing Project, an employee was ripping shingles from the roof deck with no fall protection system in use.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device was not provided to assist employees in mounting and dismounting the ladder. On or about 03/26/2019 at the Friendly's Restaurant Re-Roofing Project, employees were using an extension ladder that could not be extended 3 feet past the roof deck to access and descend to and from the roof decks and the ladder was not tied off and no grasping device was provided or in use.
