29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or abouty 03/26/2019 at the Friendly's Restaurant Re-Roofing Project, an employee was ripping shingles from the roof deck with no fall protection system in use.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. On or about 04/26/2019 at the Friendly's Restaurant Re-Roofing Project, an employee was using an extension ladder that could not be extended 3 feet past the roof deck to access the upper roof levels and the ladder was not tied off nor was a grasping device provided or in use.
