29 CFR 1910.178(l)(4)(ii)(B): The employer did not conduct refresher training, including an evaluation of the effectiveness of that training, to ensure that the operator had the knowledge and skills needed to operate the powered industrial truck safely when the operator had been involved in an accident:   a) Adient US, LLC, on or about March 26th, 2019, at the location of 7560 Arbor Dr., Northwood, OH, 43619: The employer did not ensure that the operator of the Toyota 8TB50 tugger, involved in a fall from accident on or about June 30th, 2018, was retrained in topics relevant to the safe operation of that particular tugger or similar tuggers prior to resuming tasked operations.  The employee continued to operate the tugger in this condition until the time of the inspection without the benefit of retraining, exposing the operating employee and employees in the area to fall from and struck-by hazards.
