29 CFR 1910.145(c)(2)(i): Caution signs were not used to warn against potential hazards or to caution against unsafe practices:  a)  Power Systems Control Shop;  Two soldering stations were located in the shop and lead containing solder was available for use. CAUTION LEAD signs were not posted to alert employees of the presence of lead.

29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not attached properly so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel:  a)  Transmission Line Maintenance Shop;  The Baldor grinders tongue guard was not installed properly.  It was not installed in a way which could be adjusted to the constantly decreasing diameter of the wheel.  This exposes employees to impalement hazards.

29 CFR 1910.305(j)(1)(i): Fixtures, lampholders, lamps, rosettes, and receptacles located within 8 feet of the floor had live parts exposed to employee contact:  a)  Substation Maintenance Shop Tool Room;  Florescent light fixtures had unguarded florescent light bulbs located approximately 7 feet 8 inches from the floor. The florescent light bulbs were not guarded to prevent accidental breakage/contact.

29 CFR 1910.1200(e)(1)(i): The employer did not compile a list of the hazardous chemicals known to be present using a product identifier that was referenced on the appropriate safety data sheet:  a)  Substation Maintenance Shop;  Hazardous substances including but not limited to:  Certified Labs Kool-Spray, Fabulous Blaster, Drummond 3-2-1 Electric Contact Cleaner, and Rustoleum Spray Paint were not on the inventory list.   b)  Substation Maintenance Shop Tool Room; ZEP Industrial Purpose Cleaner, Degreaser was not on the inventory list.   c)  Transmission Line Maintenance Shop;  Hazardous substances including but not limited to:  Simple Green, Henry Fibered Black Roof Coating, Chemsearch NC-1,2,3 Moisture Displacement for Electrical Equipment,  CRC Brakleen, and Fluorocarbon Dry Lubricant and Releasing Agent were not on the inventory list.   d)  Power System Control Office;  Hazardous substances including but not limited to:  Tech Spray Triple Core Solder, Kester Solder, Chem-wick lite SD,  Solder-wick, and Professional Tite bond Original Wood Glue were not on the inventory list.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  a)  Substation Maintenance Shop;  A field sample of mineral oil from transformers in a glass bottle was not labeled.    b)  Power System Control Office;  A plastic bag containing several different types of solder was not labeled.
