29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.    At the residential work site located at 1950 S Scenic Ave, Building C, Springfield, Missouri:  Employees egaged in roofing activities while using a pneumatic nail gun were not protected from flying particles by using eye or face protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    At the residential work site located at 1950 S Scenic Ave, Building C, Springfield, Missouri:  Employees were accessing a working surface on a pitched roof by way of a portable ladder whose side rails did not extend 36 inches above the upper landing surface being accessed.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     At the residential work site located at 1950 S Scenic Ave, Building C, Springfield, Missouri:  Employees engaged in roofing activities were exposed to fall hazards greater than 26 feet from the lowest point of the roof to the ground while not utilizing fall protection systems.     Raymundo Duran Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 991492, citation number 1, item number 1 and was affirmed as a final order on 10/01/2014, with respect to a workplace located at 5824 Yampo St., Aurora, CO  80015.
