29 CFR 1910.1200(g)(8): The employer did not ensure that material safety data sheets were readily accessible to the employees in their work area during each work shift:  Location:  Century 4   1) The employer did not ensure that Safety Data Sheet for the original floor stripping product, EXPOSE, used on the evenings and following mornings of March 4th and 5th, 2019, was readily accessible to the employees working on Century 4.  2) After becoming aware of an additional floor stripping product, BARE BONES LOW ODOR, that was mistakenly used, the employer never provided the SDS to the employees in the Century 4 unit.  3) The employees in the Century 4 unit did not know how to access the SDSs for the chemicals used by the hospital through the Dolphin database found on the hospital  intranet.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:   Location: Century 4  Employees were not provided effective information and training on the original floor stripping product used, Expose, when it was introduced into the work area and used at approximately 8:30 PM to 5:30 AM, on March 4th-5th, 2019.
