29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:  On or about March 26, 2019, at 4241 Folsom Avenue, Saint Louis, Missouri, the employer did not ensure that energy control procedures clearly and specifically outlined the steps for shutting down, isolating, blocking and securing machines or equipment in the following instances:  (1) There was no specific procedure for the LIQUI-BOX 1500, Fully Automatic Strip Bag Filler (LB, Pacesetter II;  Model: 1500 C1T; Energy Sources:  Electrical & Pneumatic). The procedure was not written in sufficient detail that provided enough direction for employee(s) to safely perform servicing or maintenance work on the machine.   (2)  There were no specific procedures for the Autokap Flexible Bags Filling System [DS Smith, Model: 1210;  Serial Number: 2480(TA 1879971-01); MFG:  March 2016; Romeoville, IL; Power: 120VAC - IPH-10A].  The procedure was not written in sufficient detail that provided enough direction for employee(s) to safely perform servicing or maintenance work on the machine.

Upon request, the employer was not able to provide the documentation that showed the name of the operator, the date of training, the date of the evaluation, and the identity of the person(s) who performed the training or evaluation.
