29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: (a) On or about March 11, 2019, and at times prior, the employer did not ensure that employees conducting work operations on the loading dock at 7745 Wadsworth Blvd., Arvada, CO 80003, such as disposing of trash and recyclable materials were protected from fall hazards, in that an employee fell approximately 4 feet onto the ground, sustaining head injuries. Abatement note: Abatement certification and documentation are required for this item (See Certification of Corrective Action Worksheet).
