29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems. Location: Jobsite On or about March 27, 2019 an employee was exposed to cave-in hazards while working in an excavation with unsupported vertical walls where the competent person failed to conduct a daily inspection.

Location: Jobsite On or about March 27, 2019 workers were exposed to a fall hazard of over 7 feet to the ground below while accessing the main entrance to the worksite, adjacent to an excavation, where a guard rail had not been installed.
