29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Innova Solutions LLC, 3446 Guinea Way, Castle Rock CO 80109: On or about March 6, 2019, and at times prior, employees were exposed to fall from height hazards while not wearing adequate fall protection when working at heights greater than 6 feet. Innova Solutions LLC employees were observed walking on the ceiling trusses of the third story without a personal fall arrest system consisting of a harness, lanyard and anchor point. Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).
