29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary. New residential construction located at lot E311, West side: On or about 3/25/2019, and at time prior, the employer exposed employees to falls of 13-feet 6-inches to the lower level, in that the tubular welded scaffold work platform was not fully planked between uprights creating a fall hazard to employees installing custom stonework.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: New residential construction located on Lot #E311, West side: On or about 3/25/2019, and at time prior, the employer exposed employees to falls of 13-feet 6-inches to the lower level, in that guardrails were not installed along all opens sides of the scaffold walking/working surface while employees were engaged in masonry activities.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access. Residential home located at lot E311 215 Central Ave. Fayetteville, GA. grounds: On or about 3/25/2019, and at time prior, the employer exposed employees to fall hazards of 13 feet 6 inches, in that a ladder was not provided for safe access/egress to the scaffolding system. Cross braces were used as a means of access. G & H Masonry of Georgia Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(1) , which was contained in OSHA inspection number 1314597 citation number 1 item number 1 and was affirmed as a final order on 1/04/2019, with respect to a workplace located at 290 Heatherdon Ave. Fayetteville, GA 30214.
