On or about 03/26/2019, at the job site - employees were exposed to forklift related hazards such as falling or being ejected from the forklift, in that, the forklift was not removed from service after defects were found, and was been operated while the seat belt was missing. b. On or about 03/26/2019, at the job site - employees were exposed to forklift related hazards such as struck-by, in that, the forklift was not removed from service after defects were found, and was been operated while the horn was not functioning.
