29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: (a) Excavation located at the intersection of 15th Avenue and 51st Street, Columbus, GA: On or about 03/26/2019, and at times prior, the employer exposed employees to entrapment hazards while working in an excavation which measured 13-feet, 10-inches wide by 25-feet long by 7-feet, 3-inches deep, in that a safe means of egress (i.e. ladder and/or ramp) was not provided in the excavation in the location where employees were engaged in sewer line installation activities.
