29 CFR 1910.106(e)(6)(ii): Category 1 or 2 flammable liquids , or Category 3 flammable liquids with a flashpoint below 100 degrees Fahrenheit (37.8 degrees Celsius) were not dispensed into containers unless the nozzle and the container were electrically interconnected.  Where the metallic floorplate on which the container stands while filling is electrically connected to the fill stem or where the fill stem is bonded to the container during filling operations by means of a bond wire, the provisions of this section shall be deemed to have been complied with.  Outdoor Chemical Storage Room:  Employees were exposed to fire hazards, as three 55 gallon drums of Isobutyl Alcohol, and one 55 gallon drum of Acetone were not bonded and grounded.

Employees were exposed to Isobutyl Alcohol and there was not an eye wash to provide a 15 minute flow of liquid. There was a small eyewash bottle station which was empty and would not have provided for adequate flushing of the eye.

The fire extinguisher had last been inspected in June 2017.

The gray saw in the maintenance area was not adequately guarded.  Under the table, the saw blade was not guarded on the north side.  b) Employees were exposed to amputation hazards. The New England Butt Company Braiding Machine BR5 was not guarded on the left and right sides facing the machine and across the top from the rotating spools.  c) Employees were exposed to amputation hazards.  The New England Butt Company Braiding Machine BR4 was not guarded across that top of the machine from the rotating spools.

Employees were not trained on the hazards of chemicals when they are exposed to acetone and isobutyl alcohol.  Both acetone and isbutyl alcohol are flammable chemicals.  Two or more employees had not received hazard communication training.
