29 CFR 1910.219(i)(2): Shaft couplings were not constructed as to present no hazard from bolts, nuts, setscrews, or revolving surfaces: #47 Salvage Winder - Operators were not protected from protrusions extending beyond the flange of the coupling and from the revolving surface itself as evidenced by an operator getting "caught in the winder".
