29 CFR 1926.451(e)(1): Portable ladder, hook-on ladder, attachable ladder, stair tower, stairway-type ladder, ramp, runway, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used to access scaffold platforms more than 2 feet above or below a point of access: a) On March 26, 2019, employees were not provided safe means of access between scaffold platforms where the distance between platforms was greater than 2 feet. A. Horn, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(1) which was contained in OSHA inspection number 1128095, citation 1, item 1, and was affirmed final order on April 6, 2016, with respect to a workplace located at 303 E. Olive St., Arlington Heights, IL 60004. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

A. Horn, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1) which was contained in OSHA inspection number 1128095, citation 1, item 2, and was affirmed final order on April 6, 2016, with respect to a workplace located at 303 E. Olive St., Arlington Heights, IL 60004.
