29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief was provided to prevent pull from being directly transmitted to joints or terminal screws:  Employees were exposed to electrical shock hazards.  Employee(s) engaged in electrical work at 13300 Switzer Road, Overland Park, Kansas had access to a 25 foot extension cord that was not provided strain relief to prevent the insulating outer jacket from pulling away from cord plug end.
