29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     Residential home located at 1701 Ossahatchie Creek Rd., Ellerslie, GA. Roof: On or about 3/25/2019, and at time prior, the employer exposed employees to falls of 9-feet, 2-inches to the lower level, in that employees were not provided with a fall protection system when engaged in roofing activities on a 6/12 pitched roof.

29 CFR 1926.552(a)(1): The employer did not comply with the manufacturer's specifications and limitations applicable to the operation of all hoists and elevators.                                                                     Residential home located at 1707 Ossahatchie Creek Rd. Ellerslie, GA., South side of residential structure: On or about 03/27/2019, and at times prior, the employer allowed employees to use the TranzSporter (Model LH200) Platform Hoist as a ladder to access the roof. Manufacturers general safety instructions state that this hoist should never be used as a ladder. Employees were exposed to falls as the first 36-inches of the platform hoist are obstructed by the platform and motor mounts.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  Residential home located at 1701 Ossahatchie Creek Rd. Ellerslie, GA., Roof: On or about 03/25/2019, and at times prior, the employer failed to train employees to recognize fall hazards, in that employees were not provided training to minimize fall hazards while working on 6/12 pitched residential roof.
