29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary:  On or about April 5, 2019, an employee was exposed to a fall greater than 10 feet to the ground below while attaching Tyvek insulation to a commercial construction from a pump jack scaffold that was secured to the structure with wooden boards.

29 CFR 1926.501(b)(10):  Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:  On or about April 5, 2019, employees were exposed to a fall hazard greater than 6 feet to the ground below while installing roof decking on a commercial construction without fall protection.   A-1 Framing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1341279, citation number 1, item number 4, and was affirmed as a final order on November 6, 2018, with respect to a workplace located at 22769 Estacado St., San Antonio, TX 78261.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails were not extended at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  North side of building.  On or about April 5, 2019, employees were exposed to a fall hazard while accessing the roof without the aluminum ladder side rails extending at least three feet above the landing surface.   A-1 Framing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1060(a)(1)(iii), which was contained in OSHA inspection number 1341279, citation number 1, item number 5, and was affirmed as a final order on November 6, 2018, with respect to a workplace located at 22769 Estacado St., San Antonio, TX 78261.
