29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) Richland Laminated Columns, LLC. located at 8252 OH 13 in Greenwich, Ohio; On or about March 28, 2019 the employer did not ensure an energy control program consisting of machine specific control procedures, employee training, and periodic inspections was developed and implemented. Employees tasked with cleaning the tar spray nozzles on the Green Post Machine, Super Winder, with the benefit of energy isolation, hazardous energy recognition training, and employer evaluation, were exposed to caught in and struck hazards in the event of unexpected start-up.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Richland Laminated Columns, LLC. located at 8252 OH 13 in Greenwich, Ohio; On or about March 28, 2019 the employer did not ensure employees were protected from ingoing nip points created along the automated roller-press arm located on the Green Post Super Winder. Employees were exposed to caught in and amputation hazards when entering the gated off area of the Super Winder to flatten out the polyethylene wrap on the wooden columns.

Employees operating the CAT and Toyota forklifts were not provided with training consisting of formal instruction, practical training, and an evaluation in operations. The employees operating and others working around the forklift were potentially exposed to struck-by injuries due to operation by an untrained operator.
