29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. (a) Warehouse - On or about March 28, 2019 the employer failed to report the hospitalization of an employee within 24 hours. The employee was operating a powered industrial standup reach lift when it struck a warehouse rack. The employee was hospitalized on March 22, 2019. The employer reported the hospitalization on March 25, 2019.
