29 CFR 1926.20(b)(2):  It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part; such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.    On or about March 28, 2019 at Lando Street and Hwy 67 in Plymouth, the employer did not ensure that the competent person conducted frequent and regular inspections of the job site to addressed and correct recognizable hazards associated with  wood frame construction.  Employees were exposed to fall hazards greater than six feet to lower levels, falls from ladders, and working from inadequate scaffold working platform.

29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and walkway shall be at least 18 inches (46 cm) wide.  On March 28, 2019 at Lando Street and Hwy 67 in Plymouth, an employee was working from a twelve inch wide scaffold platform at nine and one half feet above surface level.

29 CFR 1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided.  On March 28, 2019 at Lando Street and Hwy 67 in Plymouth, an employee was using a ten ft. step ladder to access/egress the top of a wall plate twelve and one half feet above surface level; break in elevation greater than two feet.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab-rail, shall be provided to assist employees in mounting and dismounting the ladder.  On March 28, 2019 at Lando Street and Hwy 67 in Plymouth, an employee was using an eight ft. step ladder to access/egress a scaffold work platform nine and one half feet above surface level without any grab-rail above the upper landing surface.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    On March 28, 2019 at Lando Street and Hwy 67 in Plymouth, an employee was working from a wall top plate and top of joists at greater than twelve feet above surface level without utilizing fall protection equipment.
