29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. a) On or about March 14, 2019, a worker was exposed to a 40 foot fall while standing on a 2 foot wide steel beam flange and signaling crane without fall protection. No abatement certification or documentation required for this item.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: (a) Coffer Dam - On or about March 27, 2019, a step constructed of a horizontal steel angle which was located 25 inches above the ground where workers used that step to enter a coffer dam. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.1416(d)(3)(ii)(B): Lattice boom cranes and derricks manufactured after November 8, 2011 was not equipped with a device which automatically prevents damage and load failure from contact between the load block, overhaul ball, or similar component, and the boom tip (or fixed upper block or similar component). The device(s) must prevent such damage/failure at all points where two-blocking could occur. (a) Coffer Dam and Staging Area - On or about March 27, 2019, workers were exposed to a struck-by hazards for the Link Belt lattice boom truck crane (serial number #N3K4-4353) suspending an overhead loads was not provided with a working anti-two block device. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
