29 CFR 1910.252(b)(4)(iv): Lifelines. Where a welder must enter a confined space through a manhole or other small opening, means shall be provided for quickly removing him in case of emergency. When safety belts and lifelines are used for this purpose they shall be so attached to the welder's body that his body cannot be jammed in a small exit opening. An attendant with a preplanned rescue procedure shall be stationed outside to observe the welder at all times and be capable of putting rescue operations into effect. a. On or about March 28, 2019, for employees who were welding inside tanks, the employer did not provide a means to remove the employee from the tank in the event of an emergency or an attendant to observe the welder. Rhodes Manufacturing was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1910.146(c)(4), which was contained in OSHA inspection number 1107120, citation number 1, item number 4b and was affirmed as a final order on May 27, 2015, with respect to a workplace located at 7045 Buckeye Valley Road, Somerset, OH 43783.

29 CFR 1910.134(c)(2)(ii): In addition, the employer must establish and implement those elements of a written respiratory protection program necessary to ensure that any employee using a respirator voluntarily is medically able to use that respirator, and that the respirator is cleaned, stored, and maintained so that its use does not present a health hazard to the user. Exception: Employers are not required to include in a written respiratory protection program those employees whose only use of respirators involves the voluntary use of filtering facepieces (dust masks). a. On or about March 28, 2019, the employer did not ensure that each employee who was using a tight-fitting respirator voluntarily received a medical evaluation.
