29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed:      On, or about March 29, 2019, at the Fairbridge Inn located at 28600 Ridgehills Drive, Wickliffe, Ohio 44092, the employer did not verify, through a written certification, that the required workplace hazard assessment had been performed when employees worked with GLB down, a corrosive pool treatment chemical that contained sulfuric acid, monosodium salt, and required the use of chemical goggles.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   On, or about March 29, 2019, at the Fairbridge Inn located at 28600 Ridgehills Drive, Wickliffe, Ohio 44092, the employer did not require employees to wear protective eye equipment such as, but not limited to chemical googles, when employees worked with chemicals such as, but not limited to GLB, pH up, which contained sulfuric acid, monosodium salt and had a pH of 0.5. Employees dispensed the product into the pool to control the pH of the pool.
