29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means.    a) Jobsite: 11601 Roosevelt Blvd, Philadelphia, Main Electric Substation - The employer permitted employees to collect scrap metal from a 13.2kv electrical cabinet without ensuring they were protected from electrical hazards by de-energizing and grounding the circuits, or by effectively guarding the circuits.  Two employees attempting to cut out a bus bar from the electrical cabinet with a sawzall suffered facial burns from a resulting arc blast.      Noted on or about 3/29/19.

29 CFR 1926.850(c): All electric, gas, water, steam, sewer, and other service lines were not shut off, capped, or otherwise controlled, outside the building line before demolition work was started. In each case, any utility company which was involved was not notified in advance.  a) Jobsite: 11601 Roosevelt Blvd, Philadelphia, Main Electric Substation - The employer did not ensure that all electric service lines outside the building were shut off before demolition work started exposing employees scrapping a 13.2 kv electrical cabinet to electrocution and shock hazards.  Noted on or about 3/29/19.

29 CFR 1926.850(a): Prior to permitting employees to start demolition operations, an engineering survey was not made by a competent person of the structure to determine the condition of the framing, floors, and walls, and possibility of unplanned collapse of any portion of the structure, including adjacent structures where employees may be exposed. The employer did not have in writing evidence that such a survey was performed.    a) Jobsite: 11601 Roosevelt Blvd, Philadelphia - The employer did not have a competent person complete an engineering survey on the approximate 400k square foot single story commercial building prior to permitting employees to conduct demolition operations, thus exposing employees to hazards such as unsafe framing, floors, and walls, and unplanned collapses.    Noted on or about 3/29/19.
