29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of �1926.502(k) developed and implemented:      a) Jobsite, 183 London Towne Drive, Pittsburgh, PA 15226:  On or about March 13, 2019, the employer did not ensure that each employee engaged in roofing work on a townhouse was protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  An employee installing shingles on the roof of the townhouse was exposed to a fall hazard of approximately 20 feet to the ground below.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device was not provided to assist employees in mounting and dismounting the ladder:     a) Jobsite, 183 London Towne Drive, Pittsburgh, PA 15226:  On or about March 13, 2019, a portable extension ladder used to access the roof of the townhouse was not secured at its top to a rigid support.   An employee using the ladder was seriously injured after falling off the ladder when the ladder slipped and fell to the ground.

29 CFR 1926.1060(a)(1)(iii): The employer did not ensure that each employee using ladders and/or stairways had been trained by a competent person in the proper construction, use, placement, and care in handling the ladders and/or stairways:  a) Jobsite, 183 London Towne Drive, Pittsburgh, PA 15226:  On or about March 13, 2019, the employer did not ensure that each employee using a portable extension ladder to access the roof of a town house was trained by a competent person in the proper use and placement of portable ladders.  The ladder used to access the roof of the townhouse slipped and fell causing an employee who was using the ladder to fall to the ground.
