29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems:       a) On or about April 1, 2019, employees were exposed to the hazard of falling approximately 16 feet while hoisting materials and equipment to the edge of the roof deck where fall protection was not provided. 29 CFR 1926.502(f)(2)(ii): Warning line systems were not rigged and supported in such a way that its lowest point (including sag) was not less than 34 inches from the walking/working surface:     a) On or about April 1, 2019, a partial warning line system positioned along the edge of the roof deck was not rigged and supported to prevent sagging below 34 inches where employees were exposed to a fall hazard of approximately 16 feet during the performance of roofing work activities.  Sections of the warning line system sagged to the roof deck.
