29 CFR 1926.651(i)(3): Sidewalks, pavements, or appurtenant structures had been undermined and a support system or another method was not provided to protect employees from the possible collapse of such structures: Jobsite: Employees were using a Takeuchi TB175 excavator to remove soil around an underground storage tank where the sides of the excavation were not adequately sloped to prevent undermining of the pavement. The undermined pavement collapsed causing the excavator to fall in the excavation.
