29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury: a) JDS Construction, Inc., 330 Dewey Ave, Boulder, CO 80304: On and before March 26, 2019, the employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. An employee experienced an amputation of his thumb while cutting siding with a 12" Dewalt compound miter saw. The materials being cut were not secured and clamped down. This condition exposed employees to an amputation hazard.

29 CFR 1926.403(g): Electrical equipment which was used did not have other markings giving voltage, current, wattage, or other ratings as necessary: a) JDS Construction, Inc., 330 Dewey Ave, Boulder, CO 80304: On and before March 26, 2019, the employer did not ensure that the 12" Dewalt compound miter saw included markings giving voltage, current, wattage, or other ratings as necessary. The saw was missing its manufacturer's label. This condition exposed employees to an electrical hazard.
