Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by stage decks:      a) ArtsQuest Center: On or about March 14, 2019, and at times prior, employees were exposed to being struck by falling decks and adjustable legs during assembly and disassembly of stages.       Feasible means of abatement include, but are not limited to, following the manufacturers Staging Dimensions Assembly Instructions For Staging Dimensions Portable Staging Product and Accessories:      1. Ensuring that damaged components are not installed.      2.  Ensuring fixed or adjustable legs are inserted into the pocket and the thumbscrew is tightened until secure.       3. Sliding the deck away from the adjacent decks sufficiently enough to confirm that they are not stuck together prior to lifting during disassembly.      4. Checking that the thumb screws are tightened prior to lifting the decks during both assembly and disassembly.       5. Implementing procedures to periodically identify and repair/replace decks that have problems with the thumbscrews not being able to be tightened and secured.      Additional means of abatement include:      6. Following the guidance for safe erection and dismantling of temporary platforms found in American National Standard Institute (ANSI) ES1.19  2018.    7. In situations where uplift can occur and potentially displace the scaffold support structures, the support structures shall be locked together vertically by pins or equivalent means per the guidance found in 29 CFR 1926.452(c)(4).
