29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: a) Jobsite: On April 2, 2019 and times prior thereto the sub-contractor employees used a portable power distribution unit, "spider box", in a manner not recommended by the manufacturer. The employees plugged into an energized spider box that was hanging from a temporary power pole. Note) Certification of corrective action is required for this item.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: a) Jobsite: On April 2, 2019 and times prior thereto the sub-contractor did not protect their employees from using unsafe electrical equipment. Employees used a portable power distribution unit, "spider box", hanging by a power cord from a temporary power pole. Strain relief was not in place on both ends of the power cord. Note) Corrected during inspection.
