29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:  (a) On or about October 30, 2019, the employer failed to submit documentation and certification of corrective action for citation #1, Item #1b issued on June 27, 2019 with an abatement due date of July 10, 2019.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) On or about 04/02/2019 - 1911 Martin Street South, Pell City, AL, employees removing shingles on a steep-sloped (6:12) roof were exposed to falls of approximately 12 feet.   Bulfrano Mendez Dba Jose Mendoza-Gomez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1221080, citation number 1, item number 3a and was affirmed as a final order on 05/09/2017, with respect to a workplace located at Lot #88, 5909 Heritage Circle, McCalla, AL 35111.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) On or about 04/02/2019 - 1911 Martin Street South, Pell City, AL, employees exposed to falls of approximately 12 feet while removing shingles from a steep-sloped roof had not been trained.
