29 CFR 1910.333(a)(1): Live parts to which an employee would be exposed were not deenergized before the employee worked on or near them, unless the employer could demonstrate that denergizing introduced additional or increased hazards or was infeasible due to equipment design or operational limitations: (a) Facility, Galvanized A Department, Between Columns L74 and L76: On or about March 28, 2019 at about 1:30 p.m., all electrical parts inside the 480 v AC, three phase, fused electrical disconnect box that an employee was working on were not deenergrized before the employee began his work inside the box nor did the employer demonstrate that deenergizing introduced additional or increased hazards or was infeasible due to equipment design or operational limitations.. (b) Facility, Galvanized A Department, Between Columns L74 and L76: On or about March 28, 2019 at about 2:00 p.m., all electrical parts inside the 480 v AC, three phase, fused electrical disconnect box that an employee was working on were not deenergrized before the employee began his work inside the box nor did the employer demonstrate that deenergizing introduced additional or increased hazards or was infeasible due to equipment design or operational limitations.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not using electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed: (a) Facility, Galvanized A Department, Between Columns L74 and L76:`On or about March 28, 2019 at about 2:00 pm an employee was working (testing voltage with a Fluke meter) in a partially energized 480 v AC, three phase, fused electrical disconnect box and he was not wearing appropriate gloves for electrical arc flash protection.
