29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:   On or about April 2, 2019, at a job site located at 4932 Portage St NW, N. Canton, Ohio 44720; during the construction (bridging and detail work) of a new commercial store , the employer did not ensure all employees using an aerial lift were protected from fall hazards approximately 16 feet by a fall protection device.
