29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, or uprights did not bear on base plates, mud sills or other adequate firm foundation: (a) Strong Framing Co. at 2870 Duke Cir. Boulder, CO 80303: On or before 4/2/19 the correcting employer did not protect subcontractor employees from injury in that poles supporting a pump jack scaffold did not bear on an approved firm foundation. The ground onsite was soft and muddy and did not adequately support the scaffold poles. This condition exposed subcontractor employees to an unsafe scaffold and fall hazards.

Each pump jack bracket did not have two positive gripping mechanisms to prevent any failure or slippage: (a) Strong Framing Co. at 2870 Duke Cir. Boulder, CO 80303: On or before 4/2/19 the correcting employer did not protect subcontractor employees from injury in that the pump jack scaffold on the side of the house was secured by a home made wooden bracket. This condition exposed subcontractor employees to an unsafe scaffold and fall hazards.

Boulder, CO 80303: On or before 4/2/19 the correcting employer did not inspect the pump jack scaffold provided to subcontractor employees to install siding on a new single family home. The scaffold was lacking in structural components and was in use despite those deficiencies. This condition exposed subcontractor employees to an unsafe scaffold and fall hazards.

The scaffold was erected by subcontractor employees who lacked training in the recognition of hazards. The pump jack scaffold was missing key structural and safety components. These conditions exposed subcontractor employees to an unsafe scaffold and fall hazards.

The scaffold deck was approximately sixteen feet above grade. This condition exposed subcontractor employees to a fall hazard of approximately sixteen feet.
