Boulder, CO 80303: On or before 4/2/19 the creating and exposing employer did not protect employees from injury in that employees installing siding on a pump jack scaffold were not using one or more methods of approved fall protection. The scaffold deck was approximately 16 feet above grade. This condition exposed employees to a fall hazard of 16 feet.

Boulder, CO 80303: On or before 4/2/19 the creating and exposing employer did not ensure that scaffolds were erected, moved, dismantled, or altered by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection. Employees had erected a pump jack scaffold that was used without guardrails. The scaffold deck was approximately 16 feet above grade. This condition exposed employees to a fall hazard of 16 feet.

Boulder, CO 80303: On or before 4/2/19 the creating and exposing employer did not ensure that employees were trained in the recognition of hazards in scaffolds. Employees were working on a pump jack scaffold and were not using one or more methods of approved fall protection. The scaffold was not erected correctly and was missing structural components. The scaffold deck was approximately 16 feet above grade. This condition exposed employees to a fall hazard of 16 feet.
