29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) Frigo System - On or about 7/9/2019, the employer failed to provide an emergency shower or emergency eyewash basin for employees exposed to anhydrous ammonia, a corrosive chemical. Anhydrous ammonia can cause severe eye or skin irritation if an employee were to be working in an area where anhydrous ammonia could be inadvertently released, or during maintenance activities such as, but not limited to, oil draining.

1910.119(j)(5): The employer did not correct deficiencies in equipment that are outside acceptable limits before further use or in a safe and timely manner when necessary means are taken to assure safe operation: (a) On or about 7/9/19, the employer failed to correct the following Frigo System deficiencies observed: 1. Employees were exposed to a slipping / fall hazard because the machinery room floor was not kept dry, free of water and other slippery liquids such as oil; and 2. Employees were exposed to an anhydrous ammonia chemical hazard with the potential for serious injury as damaged pressure vessel vapor barrier insulation was not repaired.
