29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) North side of building, garage area, on or about 4-2-19: Employees were using a portable aluminum extension ladder for roof access/egress, with side rails that were 15 inches above the landing surface, exposing the employees to a fall hazard of 9 feet 5 inches to the ground below.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Entire roof, on or about 4-2-19: Two employees were working on a sloped roof (7:12 pitch) and were not protected from falls, exposing the employees to a fall hazard of 9 feet 5 inches to the deck or 10 feet 10 inches to the ground.
