29 CFR 1910.23(e)(1)(v): Unless otherwise specified in this section, mobile ladder stands and platforms with a top step height of 4 feet (1.2 m) or above did not have handrails with a vertical height of 29.5 inches (75 cm) to 37 inches (94 cm), measured from the front edge of a step. Removable gates or non-rigid members, such as chains, may be used instead of handrails in special-use applications: a. 1000 Laval Blvd., Lawrenceville, GA (Warehouse) - Employees were exposed to fall hazard. Employees were not utilizing the mobile ladder stand in a safe manner when accessing the mobile ladder. The 15 step risers were not equipped with handrails along the side to provide employees with hand hold support, on or about April 3, 2019.

The employees are required to access the upper level storage area to retrieve and/or add storage material up on the upper level area, without a fall protection system. 29 CFR 1910.157(e)(1): The employer was not responsible for the inspection, maintenance and testing of all portable fire extinguishers in the workplace: a. 1000 Laval Blvd., Lawrenceville, GA (Throughout Warehouse) - Employees were exposed to fire hazard. The fire extinguisher located throughout the warehouse last inspection was 2005 and 2016. Also, there was fire extinguishers that indicated it needed to be recharge. The portable fire extinguishers were not being properly maintained in a operable condition, in the event of an emergency, on or about April 3, 2019.
